- Field 1: Enter the full name and address of the exporter.
- Field 2: Here you should always enter “European Community” or its English equivalent “The European Community” on the first line. On the second line, enter the country with which the EU has concluded a free trade agreement. If the agreement was negotiated before 2011, enter formally “European Community”.
- Field 3: This field is not mandatory, but if filled in, the full name and address of the recipient must be provided.

- Field 5: The name of the recipient country.
- Field 6: Information about the transport can be filled in here, but it is not mandatory.
- Field 7: Any remarks are made here according to the certificate's rules.
